Birthday Cards uses the 'stationery' feature built-in to Apple Mail, so you'll need to use Apple Mail to compose and send your messages.

Preview and choose a card in the Birthday Cards app, then send it to Apple Mail. Once you have opened a card in Apple Mail, you can use all of Mail's features to create your message.
